Paperwork/Tryout Meeting

The initial paperwork/tryout meeting for the OM Varsity Golf Team (Boys/Girls) will take place on Thursday, August 15, 2019 at 9:00 am in the Oakland Mills High School Cafeteria. Please bring all necessary paperwork required for participating in Howard County High School Athletics. You can find links to all the paperwork above by clicking on the Documents/Paperwork link.

We will be collecting paperwork and giving out information regarding the upcoming golf season and the tryout process.

Paperwork Needed:
– Parent Permission (signed)
– Physical Form (dated after April 1, 2019)
– Copy of Birth Certificate (not original)
– Concussion Info.
– Concussion Release
– Sudden Cardiac Arrest
– Proof of Residency (BG&E Bill)
